    Please fill out this form as completely and accurately as possible so we can best facilitate your refill.

    Please allow up to two business days for your prescription request to be reviewed, filled, and available for pick-up. If, for some reason, the prescription cannot be refilled, someone from our staff will contact you. For safety, we review your pet’s records before refilling a prescription.

    Prescription Drug Laws and Regulations

    • All prescriptions and medication refills must have a valid veterinarian-patient relationship according to VA state regulations.
    • Due to State Pharmacy Board regulations, we are unable to accept any returns on medications taken from Old Dominion Animal Health Center.
    • Controlled drugs prescribed for more than 7 days will require the owner’s information and birthdate to be recorded with the Prescription Monitoring Program (PMP) under VA laws.
